The name Accidental Perplexity is a mashup of two terms. Accidental Complexity is a term for complexity in a design that was introduced by the creator of the design, rather than by the essential properties of the underlying problem. Perplexity is a mathematical measure of uncertainty in how to interpret something ambiguous.
